Children are encouraged to submit their required immunizations and physicals prior to the start of the school year and no later than October 15 of the current year, or they will face expulsion from school.  Here you will find the information and necessary forms.

Students entering kindergarten, sixth and ninth grade in the 2009-2010 school year are required to submit proof of a current physical examination and immunizations to the school nurse at the start of the school year.  In addition, kindergartners and any student enrolling for the first time in any grade are now required to submit a full vision examination.

Students entering kindergarten, second and sixth grade must have a dental examination completed and submitted to the school nurse.

If your family does not have health insurance, you may be eligible for AllKids or Family Care.  Please contact the Chicago Public Schools Children and Family Benefits Unit at 773-553-1809 to learn if you qualify.  To locate a Health Care Clinic nearest you that offers low cost physical exams you may call 3-1-1.  The public heatlh clinic closest to Ravenswood is Uptown Neighborhood Health Center, 845 W. Wilson 312-744-2137. 

In addition, if you are self-employed or your current insurance does not cover immunizations for your children, be aware that by Illinois Law, immunizations are free for children under the Illinois Vaccines For Children (VCF) Program.  For more information, click here.
